Kindred Home Care is a self-initiated concept: an invented home-care provider proposing a discharge partnership to an invented hospital. The eight-slide proposal is warm without being soft. Literata headlines and italic kickers read like a handwritten letter, a small house mark repeats as the icon system, and window-light photography keeps the focus on home rather than the ward. The proof slide compares readmissions, and prices are set out plainly in one table.

Concept note: Kindred Home Care and Eastbay General Hospital are invented, and this was not a client commission. Pilot results, staffing and prices are concept data flagged in the speaker notes, not clinical evidence.
